Healthcare Professionals - Referral Process
If you are a health professional you can refer to all forms of our care (IPU stay, outpatient review, home visit, social care assessment, Spring Centre health & wellbeing services).
How to refer to The Hospice of St Francis?
Please download our standard referral form above. You can also email us at hospiceofstfrancis.berkhamsted@nhs.net to enquire about our services. If you are a health professional you can also ring us if you would like to discuss a referral with a member of our senior nursing or medical team.
Patients can be referred for a range of different services:
- Inpatient Unit admission
- Outpatient review by a Physiotherapist, Occupational Therapist or Doctor
- Support at Home from one of our Specialist Nurses or Doctors
- Assessment by our Supportive Care and Social Work Team
Referrals are prioritised for admission and discussed on a daily basis with the senior clinical team. All referrals are assessed by the Palliative Care Referral Centre.
Once we have received a referral, we will make direct contact with the referrer to give an update on when a bed might be available.
