
“The Community Team is no longer solely a nursing team. We now offer a much wider range of care as a multidisciplinary team.
We’re managing more complex symptoms and social concerns, and often need to react rapidly, especially in times of crisis.
Ten years ago, our caseloads consisted primarily of cancer patients. Now, we also support many patients with non-cancer diagnoses, including respiratory and neurological conditions, heart failure, and increasingly, end-stage frailty and dementia.
We help patients to navigate the complex NHS and social care systems. We also alleviate the fears and anxieties of patients and their loved ones. They know we’re doing our utmost to support them.
I’m immensely proud of my team. The care they deliver is outstanding.
