“I Want Heart Failure Patients To Know

Who should attend:

Open to any health or social care professional who supports individuals living with heart failure.
May be of particular benefit to GP's, Practice Nurses, Resident Doctors at NHS Trusts, Community Teams, Community Matrons or Palliative Care staff who want to learn how they can support such individuals

 

Course Aims:

To learn more about the management of heart failure and the role of palliative care to support individuals


Objectives:

  • To state the pathophysiology and types of heart failure including progression of heart failure
  • To gain insights into the patient’s experience of living with heart failure
  • To describe the role of the specialist heart failure nurses and the role of palliative care
  • To identify common symptoms in heart failure and state some drug and non-drug approaches to its management
  • To explore approaches to advance care planning including difficult conversations about managing ICDs

Dr Sharon Chadwick)

Dr. Sharon Chadwick

Medical Director & Deputy CEO

Sharon has more than 20 years experience working with consultant cardiologists and the community heart failure team looking after patients living with this illness, and helping them with best quality of life possible. This includes symptom control, psychological support and planning for the future.

Having trained in Respiratory Medicine, she has developed services locally for those suffering from advanced lung diseases and heart failure. As a result The Hospice of St Francis is recognised as a specialist centre for the palliative care of those conditions.

She is the medical and end of life Palliative Care lead at Watford Hospital and works there 2 days per week.

Increasingly Dr Chadwick is asked to teach at both local and national level.

Diagnosed with dilated cardiomyopathy and severe heart failure in 2005, Stephen experienced a ten year decline, culminating in advanced heart failure. Unsurprisingly, this journey has led him to have a keen interest in palliative care for advanced heart failure patients.

Palliative Care Heart Failure Specialist, Expert by Experience

Julie works within the community heart failure team at The Hospice of St Francis and has setup a support group to look after the most unwell patients at home thereby preventing, where possible, hospital admission. Previous to that Julie was a transplant coordinator.

Advanced Clinical Practitioner and Lead for Community Cardiology, CLCH

With a nursing career spanning more than 30 years Zoe spent many years working in intensive care before moving to cardiology working in cardiovascular risk preventions and genetic testing. Zoe moved to community heart failure in 2013.
